PROJECT DESCRIPTION
Our Continuous Care Program is a chronic care management program where patients can get personalized care, manage and share their patient records, get discounts on laboratory tests and pharmacy refills and other additional benefits. Our goal is to ease the burden of chronic care patients and give them personalized care as well as saving them the burden of costs that they encounter on medication and tests. We need volunteers to assist in engaging doctors to acquire patients and drive the education of users on the continuous care program.
Project Objectives
- Reduce hospitalisation of patients with chronic conditions
- Decrease cost of healthcare management for patients with chronic conditions
- Improve convenience of healthcare solutions
ABOUT Zuri Health
Zuri Health is a virtual hospital (Super Health App) providing affordable and accessible healthcare services to patients across Sub-Saharan Africa via mobile app, website, Whatsapp bot, and SMS service. Through our platforms, patients can chat and consult with doctors, buy medication from pharmacies, book laboratory and diagnostic tests and even have a doctor visit them at home. With Zuri Health, the under-served populace can now access affordable and sustainable healthcare via SMS service to take care of the over 65% of the population without access to smartphones or the internet.
